Summary
Professional responsibilities To maintain registration with the relevant Professional Bodies. To be responsible for individual Continuing Professional Development (CPD). Main areas of responsibility To be responsible to the Therapy Team Manager and take part in the organisation and co-ordination of an efficient and effective provision of Physiotherapy services including breathlessness clinics and manual handling training across both sites. To ensure a cohesive service by close co-operation with the Hospice team within the framework of Multidisciplinary Care and the Living Well concept To be managing a caseload of referrals received for both inpatient and outpatients by planning, prioritising and implementing physiotherapy interventions and adapting them as patients clinical presentation and needs change. To be able to thoroughly analyse complex information and to select relevant material from a range of resources. To undertake a holistic, specialised physiotherapy assessment and implement therapy intervention for complex patients with progressive and life-threatening diseases as an autonomous practitioner, in an efficient and effective manner, to optimise the patients function and independence. This includes using analytical skills, clinical reasoning and use of appropriate outcome measures to evaluate and adapt interventions as appropriate. Use different communication styles and approaches to encourage patients and their families/carers to participate in therapy, with the aim to work towards their patient centred goals To be a competent and effective sole practitioner. When appropriate, to work jointly with other disciplines to support the needs of the patient. To be able to provide emotional and psychological support to patients and carers. To assess and carry out Home Visits if appropriate. To be highly skilled in communication with all facets of the interdisciplinary team both formal and informal to ensure effective patient care, continuity and discharge. Be highly skilled in communication with patients and their families/carers in emotionally charged situations using tact and empathy to communicate difficult and challenging information. To work closely with the wider Therapy team in order to identify potential areas for service delivery improvements and to implement any such improvements in a timely manner. To act as a source of expert advice to, and integral member of, the multidisciplinary team to promote a rehabilitative approach in a palliative care setting within the concept of Living Well. To plan and facilitate activities and programmes of physiotherapy intervention to educate and support individuals or groups to alleviate symptoms in a holistic manner. To be willing to be a clinical educator for physiotherapy students. Administration To keep clear, accurate and timely patient records. To complete appropriate outcome measures. To complete all necessary mandatory training in a timely manner. To attend annual manual handling update for trainers. To attend relevant updates essential for continuing good practice. Training and development of staff To take responsibility for providing the moving and handling training of clinical staff along with colleagues. To give appropriate advice and instruction to staff on the physical management of the patient. To be prepared to give talks/demonstrations to colleagues and AHPs inside / outside the Hospice. Planning and development of service Participates in the clinical audit processes. Contributes to statistical activity and project reports as required. To contribute to and support the development and evaluation of the Physiotherapy service in collaboration with the Lead therapist and Matron To act as a representative of Severn Hospice to be aware of its charitable status to actively support and promote its development as an organisation and charity General To set a high personal standard or work. Must be car owner/driver. To be able to work flexible hours. Promote and incorporate equality and diversity in all aspects of the teams operations and in the public domain. All new Hospice employees are on a 6 month probationary period.
