Summary
The post holder will ensure the delivery of efficient and effective planning and implementation of the personalised care agenda. Accountable and responsible to the Macmillan Lead Cancer Nurse. Responsible for the Macmillan Cancer Information and Support Service and team. The post holder will be required to line manage the Macmillan Cancer Support Manager. The post holder will work with cancer multi-disciplinary teams on the delivery of the personalised care agenda. Support service leads in the delivery of service transformation and embedding of risk stratified and patient initiated follow up. Liaise with clinical and administrative teams to produce appropriate processes to support the development of the services. Liaise with primary care partners with regards to service development and potential changes within primary care. Support business process change and service improvement planning. Effective communication of projects benefits, justification and progress to all stakeholders. Raise awareness to support the delivery of the project.
