Managing a complex caseload, you will act as a keyworker, coordinating care, delivering therapeutic interventions, and providing clinical and caseload supervision to support colleagues. Within the Older Adults Team, you will assess, plan, and manage care for older people, working closely with mental health services and Staffordshire Police. Drawing on your experience in dementia and older adult care, you will complete risk assessments, develop personalised care plans, and respond flexibly in challenging and distressing situations. You will also contribute to staff supervision, support timely discharges, and ensure smooth transitions of care to optimise service user outcomes.
