Summary

The NHS England ambition for Frailty Virtual Wards is to bring care closer to home, supporting patients who would otherwise require hospital admission. This approach is gaining real momentum across Devon, and our teams are at the heart of delivering this change. Key responsibilities In this role, you will place people at the centre of everything you do, ensuring each individuals voice is heard and respected when planning and delivering their care. You will build trusting relationships with patients and their families, taking time to listen, communicate effectively, and respond with empathy in every interaction. Care will be delivered in patients own homes, always respecting their preferences, choices, and circumstances. Working collaboratively within a multidisciplinary team, you will contribute to holistic, high-quality care planning, ensuring the best possible outcomes for patients and the wider community. You will actively participate in MDT discussions, sharing insight, supporting decision-making, and contributing positively to service development and improvement. You will take responsibility for enhancing services through feedback, reflection, and a proactive approach to change, helping to shape innovative ways of delivering care that support people to live well and independently for longer. A key part of your role will include supporting and mentoring junior colleagues, fostering a positive team culture, and working together to achieve shared goals. You will act as a professional role model, recognising the value and contribution of every team member and the wider support network involved in a patients care. You will also play an important role in maintaining a safe, supportive, and high-quality clinical environment for patients, carers, visitors, and staff, while consistently upholding Trust values and professional standards.
