Summary
In this role, you will drive the use of population health data and tools to shape strategic decision making, support transformation, and strengthen the systems ability to improve outcomes for local communities. You will be:Leading the development of PHM capability across the HCP, including the training, mentoring and upskilling of colleagues. Developing and maintaining reporting infrastructure for neighbourhood and place-based outcomes. Producing insight packs, analytical briefings and predictive modelling. Providing insights to inform decision-making on system resource allocation and commissioning options. Tracking and validating system-wide outcomes. Integrating risk stratification, predictive modelling and PHM tools into clinical and operational workflows. Broadening organisational understanding of PHM methodologies and promoting best practice. Working with trust BIU teams to ensure PHM principles and tools are adopted and effectively utilised. Building strong relationships with system partners. Helping to build alignment and understanding across different organisational cultures, priorities and analytical needs.
