Summary
Key Responsibilities: Responsible for the delivery of practice nursing services, working as part of the practice team. To provide support to GPs in the management of chronic diseases such as Diabetes, COPD, Asthma, Hypertension, Heart Failure etc. by providing specifically defined clinical sessions, working closely with the GPs to manage patients overall care. To provide wound care, dressings, including pressure bandaging and higher level wound care, liaising directly, where appropriate with community and secondary care counter-parts, escalating to a GP as necessary. To undertake Cervical Cytology sampling and administer immunisations, including travel medicine and contraceptive medicine as outlined by national programmes and local guidelines, working closely with the nursing, clinical and admin team to create systems to identify patients who are in need of such services and promoting the service hard to reach patients. Manage appointments, accurate maintenance of clinical records, effective use of SNOMED codes, and the management of chronic disease registers. The responsibilities include identifying, managing, and supporting patients at risk of developing long-term conditions to prevent adverse effects on their health. Support health promotion initiatives within the practice by providing information to patients on healthy lifestyle choices and encouraging patients to engage in preventive healthcare measures. Contribute to patient education programs within the practice. Adhere to infection control protocols to maintain a safe and clean environment for patients and staff. Assist in maintaining the cleanliness and organization of clinical areas. Collaborate with healthcare professionals, including GPs, nurses, and administrative staff, to ensure smooth operations within the practice.
