Summary
For people living in BNSSG, there is a 16-year gap in the median age of death for cirrhosis and other diseases of the liver between the most and least deprived areas. This gap is more than double that of the second-largest gap, which is diabetes (a gap of 7 years). BNSSG ICB is working with Alright My Liver (hosted at UHBW) to enhance existing service provision with the aim to reduce the impact of deprivation/healthcare inequalities on liver disease outcomes in BNSSG.
The post holder will be integral in supporting the project aims which include
> Reduce the impact of deprivation/healthcare inequalities on liver disease outcomes in BNSSG.
> Access/navigate community support including other healthcare related services.
>Reduce DNA rates in outpatient
To provide project support to the UHBW Hepatitis C Operational Delivery Network and BNSSG ICB project. This will involve:
Ø Working closely with Specialist Nurses, clinicians and support agencies within the Bristol and Severn Hep C Operational Delivery Network (‘ODN’), to support the effective management of the programmes in order to proactively manage the project deliverables: Specifically, this post will enable patients admitted with alcohol related liver disease to access and navigate outpatient and community support/services.
Ø Contribute to ongoing planning of projects, to ensure that the project deliverables are met within the duration of the project.
Ø Help prepare documentation for MDT meetings.
Ø Accurately collect and record patient-level data. Maintain and enter data into local and national databases ensuring patients are seen promptly within their pathway.
Ø Managing patient recall systems.
