An exciting opportunity has arisen in the Chemotherapy Day Unit at Guys Cancer Centre for a Band 3 Patient Pathway Access Co Ordinator.

We are seeking an enthusiastic, dynamic, and innovative individual with a patient-centred approach, who enjoys working in a busy, friendly, multidisciplinary team environment to support us in the delivery of high-quality services.
Most of our care takes place in the purpose-built Cancer Centre at Guy's. Our Cancer Centre at Queen Mary's Hospital (QMH), Sidcup means we can also, treat local patients closer to home.
Flexible working - Opportunity to work from home one day a week, once fully trained and complete knowledge of service (subject to service demands).
Cross site cover as per rota permits
This is a permanent, full time role - 37.5 hours per week, alternating shift pattern: 8am - 6pm Monday- Friday - incl. Bank holidays (the occasional Saturday, subject to service needs).

The Patient Access Co-ordinator will be the first point of contact for patients and service users providing an in depth knowledge of outpatient and inpatient scheduling procedures for their speciality.
The Patient Access Co-ordinator will liaise closely with the clinical team and speciality management teams to ensure patients receive timely and
appropriate appointments according to their care pathway.
Excellent organisational and time-management skills are essential to ensure the smooth coordination of patient access, appointments and pathways of care. The post holder will play a key role in supporting patients and their families throughout their care journey, providing a professional, responsive and compassionate service at all times.
The Patient Access Coordinator will act as a central point of contact for patients, carers, clinical teams and other bodies such as GPs and Pharmacist, ensuring accurate information, timely communication and effective pathway management. A strong commitment to delivering high-quality patient care and maintaining patient confidentiality is essential.

Act as the first point of call for all patient access queries to the Trust.
Book appointments from the work queue on EPIC, in date order in line breach and target dates.
Be responsible for the scheduling of outpatient, elective admissions and all associated appointments according to clinical urgency and length of wait in accordance with departmental procedures and Trust Policies.
Liaise with patients prior to their hospital visit to ensure that a mutually convenient date and time is agreed.
Ensure that patients are appropriately reminded of their appointments. This may include sending reminder letters or contacting patients by telephone, up to seven days before their visit to re-confirm attendance in line with departmental protocols.
Ensure that departmental standards are met for patients receiving notification or acknowledgement of their visit.
Provide a courteous and efficient point of contact for patients both face to face and over the telephone.
Ensure that each patient is treated as an individual and their individual needs are met (i.e.: appropriate support is in place for their appointment, such as interpreting support).
