Summary

Role Purpose The Band 7 Community Nurse is an exceptional autonomous practitioner responsible for delivering advanced, highquality care to patients within their homes. The role plays a critical part in preventing avoidable hospital admissions and supporting safe, effective patient flow through timely assessment, intervention, and clinical decisionmaking. Key Responsibilities Provide expert clinical assessment and advanced nursing interventions for patients with complex and acute needs in their own homes. Work closely with the Hospital @ Home team to support admission avoidance and maintain effective system flow. Deliver a wide range of advanced clinical skills, including IV therapy, palliative and endoflife care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. Act as a senior clinical resource, offering expert guidance and support to colleagues in managing complex cases. Lead by example in providing safe, personcentred, holistic care that promotes independence and wellbeing. Leadership and Staff Development Provide strong clinical leadership and daytoday support for Band 5 and Band 6 nursing staff. Deliver training, supervision, and mentorship to develop the team's skills, confidence, and professional growth. Coordinate workloads, support effective team working, and ensure high standards of care across the service. Demonstrate clear evidence of effective team management and the ability to lead others in a dynamic community environment. Collaboration and Service Improvement Work in partnership with Team Leaders, GPs, AHPs, and wider multidisciplinary colleagues to ensure safe, joinedup care. Contribute to service development, quality improvement, and embedding best practice. Maintain accurate, timely documentation and uphold professional standards in line with NMC requirements. Professional Requirements Registered Nurse with extensive community nursing experience. Advanced clinical competence relevant to community practice. Evidence of team leadership, supervision, and staff development. Strong decision making skills and a commitment to continuous improvement.
