This is a joint advert with Community rotational post which is currently also out for advert.
• To undertake holistic OT assessment, clinical diagnosis and individualised treatment plans for patients on the caseload.
• To contribute to the inter-disciplinary working within the service, providing an effective, high-quality day to day service provision to the active client group.
• Rotations to include:• Farnham inpatient rehabilitation.
• Enhanced Recovery and Support at Home.
• There is potential for further rotations to be added within future scope.
• Hours include rostering for 7-day working with hours of work varying from 8-4 to 10-6 during the week, which can vary across rotations.

• Take responsibility for managing a varied patient caseload and follow the OT process, seeking advice on the more complex cases to ensure the provision of a timely, clinically effective service based on evidence-based practice.
• To provide holistic OT assessment, goal setting, individualised treatment programmes, care planning and delivery and review within intermediate care; adapting as the patients' needs evolve.
• To provide standardised cognitive assessments and treatment programmes as required.
• To provide environmental and functional assessments, composing & implementing appropriate treatment programmes.
• To educate and provide necessary interventions around management of anxiety and fatigue.
• To identify, assess and manage risk.
• Demonstrate a sound understanding of clinical risk and report untoward incidents both verbally and in writing using the Trust reporting system
• To work closely and jointly at times with Physiotherapy, Nursing colleagues and the rest of the MDT to ensure the patient receives holistic treatment.
