Summary


6-month secondment (maternity cover) Full time

We are looking for an experienced and practicing Advanced Clinical Practitioner (ACP) to join our Acute Medicine team for a 6-month secondment. This is an excellent opportunity to develop your skills and build a portfolio in acute assessment and management across a wide range of medical conditions within a fast-paced environment. This will give experience in SDEC working and understand the importance of flow and high-level decision making which are valuable to many other services across the trust.

You will work autonomously as part of a multidisciplinary team, providing advanced clinical assessment, diagnosis, and treatment. The role also involves supporting junior staff and contributing to service development.

Applicants must hold a Master’s level qualification (or equivalent) in Advanced Clinical Practice with either diploma or full degree and demonstrate competence in advanced clinical assessment and decision-making. Non-medical prescribing is essential and experience in acute care desirable.

Job Purpose:
To provide advanced clinical care within Acute Medicine, ensuring timely assessment, diagnosis, and management of patients presenting with acute medical conditions.

Key Duties:
• Perform comprehensive clinical assessments and formulate management plans.
• Prescribe medications (if qualified) and order investigations.
• Work collaboratively with consultants, nurses, and allied health professionals.
• Participate in teaching, supervision, and service improvement initiatives.
• Maintain accurate documentation and adhere to governance standards.

To be responsible for managing practice, using an advanced level of professional accountability, autonomy and judgement for highly complex decision-making across a broad range of situations/conditions.

To work using advanced knowledge and skills, underpinned by theory and experience, to holistically assess the patients using a range of methods across a broad range of, often complex, patient conditions within Acute Medicine.

To plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate, to optimise health outcomes and resource use, as well as providing direct support to patients and carers.

To propose and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care and Poole Approach. To act as a resource to others and deliver and evaluate a range of relevant specialist programmes of education and development for individuals and large groups.
