Summary

The post holder will provide specialist care for patients with Parkinsons disease and provide support and advice to the wider MDT. To manage an active caseload of people with Parkinsons and to work with patients and their carers to provide interventions and to develop self-management plans for Parkinsons to optimise health and quality of life and reduce avoidable hospital admission. To work with in collaboration with patients and their carers, GP & Hospital Consultants, to achieve optimum therapeutic benefit from medication and treatment regimen. To support the multidisciplinary team to provide specialist care to patients with Parkinsons disease. To develop and deliver education and training relating to Parkinsons Disease for the MDT, for other health and social care professionals and for people affected by Parkinsons. To share expertise which is relevant to other long-term neurological conditions with other members of the MDT where appropriate. Be an integral part of the Community Stroke and Neuro Rehabilitation Team and Neurological Conditions Specialist Practitioner team. The post holder will maintain a clinical caseload for people with Parkinsons Disease.
