Summary
Supporting admissions, ward rounds, clinical meetings and MDT discussions Providing holistic palliative care, addressing physical, psychological, social and spiritual needs Maintaining accurate contemporaneous records in SystmOne Completing medical examiner referrals, coroner referrals and MCCDs (with consultant support) Assessing patients in day services, outpatients and community settings Liaising with primary and secondary care teams to ensure continuity of care Contributing to teaching, clinical governance, audit and research Engaging in ongoing CPD and annual appraisal
