Summary

Salary: £24,915 per annum
Contract: Permanent
Reporting to: By Your Side Senior Co-ordinator
Hours: 35 per week, evening, weekend and BH working/Rota System.
Responsible for: N/A
Location: The Beehive Resource Centre, West Street, Grays, Essex
Essential Car User: Yes with Business Insurance Included
Annual Leave: 25 days annual leave plus bank holidays (+ 3 days Xmas)
Leave entitlement increases by 1 day per year (up to 30 days).

Job Overview

Would you like to finish your working day knowing you have done something to help someone in our communities?
Whether it’s calling someone that has been discharged from hospital to confirm they have support around them, helping the hospital discharge team get equipment in place to enable someone to return home or supporting someone to stay well at home and avoid admission you will be helping some of our vulnerable residents and helping hospitals with smooth, safe discharges.
By Your Side are looking for warm, caring, and empathetic team players with a real can do attitude to join our home from hospital and admission avoidance projects.

What you’ll be doing:

Duties include, but are not limited to, the following:

After training to be able to take a referral collecting all information needed and understand what BYS input is required.

As part of your role you will be required to carry out home visits for clients and complete small tasks such as; Fitting Temporary Key safes (training will be given), small tasks within the home and shopping the use of a car is essential.

Work in partnership with other voluntary and statutory organisations.

Ensure the project maintains accurate and up to date records to enable the service to be monitored and evaluated appropriately.

To undertake wellbeing calls and related admin uploading onto CRM system daily (training will be given).

Once trained to attend Multi-Disciplinary Team meetings to input on individual Residents.

To visit sites across Thurrock and attend meetings representing BYS and CVS.

Attend one to one supervision and staff meetings as arranged with the By Your Side Senior co-ordinator.

Undertake any other duties appropriate within the role as may be reasonably required from the By Your Side Senior Co-Ordinator.

Work within the policies and procedures of Thurrock CVS.

Undertake training and professional development as and when required.

Organisational Skills:

Some working knowledge of the Microsoft Packages.
Must be able to travel around the borough and able to work to a 7 day 365 day service, it operates on a rota system including evenings until 7pm and weekends.


Interpersonal and Communication Skills:

Have good communication skills.
Have a good understanding of the needs of older people.
Be able to maintain accurate records.
Ability to use a Computer including Microsoft office and email and to use BYS CRM once trained.
Ability to work as a team and alone when necessary using own initiative.
Have a flexible approach to work.
Full driving license and access to a car at all times for which Business insurance will be required.
Have experience of working with or as a volunteer.
Have experience of working in the community.
