About the role: Assess, plan, implement and evaluate individualised care using evidence based guidelines To work collaboratively with the inter-disciplinary team to deliver high quality care Maintain all records in line with corporate policies and procedures and NMC guidelines Carry out all clinical care in line with Brainkind policies and procedures Ensure the care team provide all care in line with the inter-professional evidence based care plan Where appropriate be responsible for the management of patient care during the shift
