About the Role
We are recruiting on behalf of a well-established care provider for an experienced and dedicated Care Coordinator to join their residential care home team in Woodbridge.
As a Care Coordinator, you will play a key role in ensuring residents receive high-quality, person-centred care. You will support the smooth day-to-day running of the home by coordinating care delivery, supporting the care team on shift, and ensuring care standards are consistently met.
You will work closely with the Home Manager and wider team to support staff, oversee care documentation, and ensure safe and effective care delivery.
Key responsibilities include:
• Coordinating and supporting care delivery across shifts
• Administering and recording medication
• Supporting residents with their individual care needs where required
• Updating and maintaining digital care plans
• Liaising with healthcare professionals
• Supporting induction, training, and supervision of care staff
• Promoting dignity, independence, and high standards of care
About You
• Experience supervising or supporting a care team in a residential care setting
• A caring, patient, and compassionate approach
• Strong leadership and communication skills
• Ability to work effectively as part of a team
• Good organisational skills and attention to detail
• Flexibility to work across varying responsibilities and shifts
• NVQ Level 3 in Health & Social Care (or working towards)
