Summary
Band 5 This is a development post working under the supervision of an experienced transport practitioner and medical staff. Accountable for assessing, planning, delivering and evaluating high quality patient care Provide effective support to both patient and carers throughout their care pathway. Act as role model, teacher and supervisor for junior staff and students, participating in mentor and preceptorship programmes To abide by Trust policies and procedures. Provide support for and depending on experience deputise for practitioner in charge in their absence. Act as patient advocate in accordance with Nursing and Midwifery Council (NMC) Code/Health and Care Professional Council (HCPC) Code. Work collaboratively as an effective member of MDT. Band 6 The Newcastle upon Tyne Hospitals NHS Foundation Trust defines this role as a qualified nurse/Practitioner where the primary purpose is to provide guidance and support based on advanced level knowledge to patients, carers and health professionals concerning a specific disease/condition. Within the sphere of specialist practice: Is working towards advanced knowledge and skills within specialist sphere of practice. Contributes to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients. Support the development of nursing practice within the specialist field and contribute to the body of nursing knowledge. Contributes to training and education within specialist area.
