Job Description: Advanced Nurse Practitioner (ANP) Job Title: Advanced Nurse Practitioner Employer: Harwood Group Practice Location: Harwood Medical Centre / Tonge Moor Health Centre, Bolton Salary:(Dependent on experience) Hours: 37.5 hours per week (Monday Friday, 08:00 18:30) Mandatory Requirement Independent Prescribing (V300) is an essential requirement for this role. To ensure the safety and efficiency of our autonomous clinical service, applicants who do not hold a valid Independent/Supplementary Prescribing qualification will not be considered for this position.Job Summary The post-holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. The role is split between site-based clinics and a community-based home visiting service, ensuring that patients who are housebound or in residential care receive high-standard clinical intervention. As an independent prescriber, you will be responsible for the complete episode of care, from diagnosis to issuing necessary prescriptions.Main Duties & Responsibilities1. Clinical Practice & Prescribing Autonomous Consultations: Deliver face-to-face and telephone consultations for patients with undifferentiated and undiagnosed problems. Independent Prescribing: Issue prescriptions safely and effectively in accordance with your scope of practice, national legislation, and local formularies. Medication Review: Conduct complex medication reviews for patients with multiple comorbidities, particularly during home visits. Acute & Chronic Care: Manage a wide-ranging caseload including acute minor illnesses and complex long-term conditions (COPD, Diabetes, Frailty). Home Visiting: Lead the clinical response for housebound patients, providing acute assessments and proactive frailty reviews.2. Multi-Disciplinary Team (MDT) & Leadership Collaborative Working: Work as an integral part of the Harwood MDT, participating in clinical meetings, safeguarding reviews, and GSF meetings. Referrals: Ensure timely and appropriate referrals to PCN specialists and secondary care. Support: Act as a clinical resource for the wider nursing team regarding prescribing queries and complex case management.
