Summary
The successful candidate will be an accurate, detail-focused clinical coder with experience of medical records and strong IT skills (including EMIS and Docman). They will be organised and able to manage their own workload, while communicating clearly and courteously with clinicians, administrative staff and patients. Calm under pressure and a supportive team player, they will use tact, discretion and initiative when handling sensitive information and contributing to safe, high-quality patient care. The main responsibilities include: Accurately enter SNOMED CT codes into patient medical records, ensuring the correct and most relevant codes are used. Ensure all coding complies with practice standards, contractual requirements, and practice policies. Work collaboratively with clinical staff to clarify information and resolve coding discrepancies. Maintain patient records in line with confidentiality, GDPR, and data protection requirements. Assist with audits, reporting, and data validation by providing accurate coded information. Support the practice in achieving Quality and Outcomes Framework (QOF) and other performance targets. Contribute to the continuous improvement of data quality across the practice. Provide a clear and effective handover of any outstanding matters to colleagues before starting annual leave or other planned absence. Input data from new patient health questionnaires. Process changes to patients personal details, such as address and contact numbers. Work in accordance with all Practice policies and procedures.
