Summary
Job Summary The post holder will be responsible for reviewing, summarising, and coding clinical information within patient records using SNOMED CT in SystmOne. The role supports the delivery of safe and effective patient care by ensuring that clinical data is accurate, complete, and up to date. The post holder will work as part of the Patient Services Team, contributing to efficient workflows, data quality, and reporting requirements. Key Responsibilities Clinical Coding & Record Management Review and summarise patient records and clinical correspondence Accurately code clinical information using SNOMED CT within SystmOne Ensure patient records are complete, accurate, and up to date Identify relevant clinical information and escalate where necessary Correspondence & Workflow Management Process incoming clinical correspondence (electronic and paper) Allocate and workflow information to appropriate clinicians or teams Support efficient document management processes Data Quality & Reporting Maintain high standards of data quality and consistency Support QOF, enhanced services, and patient recall systems Assist with audits, searches, and reporting requirements Contribute to data quality improvement initiatives Team Working & Communication Work collaboratively with clinicians and administrative colleagues Communicate effectively regarding coding queries or data issues Support a positive and professional team environment Information Governance & Compliance Maintain strict confidentiality of patient information Comply with GDPR, data protection, and information governance policies Ensure accurate and secure handling of all patient data General Duties Prioritise workload effectively to meet deadlines in a busy environment Maintain a high level of accuracy and attention to detail Undertake training and development relevant to the role Support colleagues and contribute to continuous improvement Carry out any other reasonable duties appropriate to the role Additional Information This job description is intended as a guide to the main responsibilities of the role and may be amended to meet the needs of the service.
