Summary
We are recruiting for a Personal Advisor to support care leavers aged 18-25 at London Borough of Hackney.
PAY RATE: £22.00 per hour (Contractor rate)
Location: Hackney Service Centre, 1 Hillman Street, London E8 1DY
Hours: Full time - Flexible hours required including early mornings, evenings and occasional weekends
Key Responsibilities:
- Establish rapport and build respectful relationships with care leavers, their families and support networks
- Undertake Review Pathway Plans and coordinate support interventions ensuring views and wishes are heard
- Engage and communicate effectively with other agencies and include professional views in plans
- Work with care leavers in their accommodation to monitor, assess risk and implement support plans
- Use task-centred approach to enable care leavers to develop independence skills
- Prepare clear and concise Review Pathway Plans based on evidence
- Attend multi-agency meetings including Looked After Children Reviews and Child Protection Conferences
- Maintain meticulous case records relating to care leaver circumstances
Requirements:
- NVQ Level 3 or equivalent child and family training
- Experience of direct work with children, young people and families
- Understanding of care leaver legislation, safeguarding procedures and pathway planning
- Ability to create rapport and build professional relationships with young people
- Computer literate with information management systems
- Enhanced DBS check required
