Our Practice Nurse will: Deliver nursing care to patients across a range of clinical areas Support the management of patients with long-term conditions Promote health and wellbeing through preventative care and education Work both independently and as part of the wider practice team Job Responsibilities Clinical Care Provide high-quality, evidence-based nursing care in line with NICE guidelines Deliver routine nursing services including: Wound care and dressings, including suture and staple removal Cervical cytology Immunisations (routine, childhood, and travel) ECGs and spirometry Undertake new patient health checks and routine screening Chronic Disease Management Support the management of patients with long-term conditions, including: Diabetes Asthma and COPD Hypertension Carry out reviews, monitoring, and patient education Support patients to self-manage their conditions We welcome applications from Nurses across all specialist fields. Health Promotion & Prevention Deliver opportunistic health promotion and lifestyle advice Support national and local health campaigns (e.g. vaccinations, NHS health checks) Promote patient wellbeing and disease prevention Infection Prevention & Control Work in accordance with infection prevention and control policies Support IPC audits and compliance activities as required Medicines Management Administer medications in accordance with PGDs and scope of practice Provide advice to patients on medication use, side effects, and compliance Patient Management & Administration Request and process pathology tests as appropriate Maintain accurate, clear, and contemporaneous clinical records Refer patients to GPs or other healthcare professionals when appropriate Team Working Work collaboratively with GPs, nursing colleagues, and the wider MDT Support colleagues and contribute to a positive team environment Participate in practice meetings and training
