Summary


We have an exciting opportunity for a caring and dynamic person to work within the Children's Diabetes Service as a Patient Advocate. The post holder will work alongside the multidisciplinary team in providing high quality patient care for children and young people with both Type 1 and Type 2 Diabetes.

The team are based at the Rainbow Community Hub in Elland and care for children in both Huddersfield and Calderdale up to the age of 18

To work holistically in the support of children and young people with diabetes and their families as an integral part of the multidisciplinary team.

This role often involves working alone in the community visiting children in their homes, in school and other settings, therefore access to own vehicle is essential

To support the Consultant, Specialist Nurses, Psychologist and Dietitian in the multidisciplinary clinic, nurse led clinic and transition clinics

Acting as an advocate for the children, young people and their families

Attending Consultant led, Nurse led and Transition clinics

Attending relevant multidisciplinary meetings to safeguard children at all times and escalating concerns appropriately

Attending evening transition clinics once a month to support the young person and ensure appropriate structured education has been given prior to discharge to young adult services

Assisting in the commencement of Hybrid Closed Loop Insulin Pumps and ensuring any technological phone App issues have been resolved beforehand

Organising social and activity events for children, young people and their families

Attending schools independently and supporting the children and young people, providing education where necessary and awareness to peers if requested

To have excellent communication skills with the children, young people, their families, work colleagues and other multidisciplinary team members

To be able to organise own workload effectively

To assist other team members to empower and motivate children and young people in the development of practical skills from diagnosis to discharge to the young adult team through structured education

To be actively involved with students during their placement within the team

To attend Team and MDT meetings

To attend relevant training where necessary
