Summary

Assess patients, including complex cases, to create and review effective treatment plans. Deliver personalised, goal-focused treatment using evidence-based practice. Work closely with the wider multidisciplinary team, including social care and voluntary services. Support patients with lifestyle changes to improve daily living, work, education, and leisure. Manage and prioritise your caseload, adapting plans and delegating tasks when needed. Motivate patients and involve families/carers in treatment. Communicate clearly, including with patients who have communication difficulties. Follow professional standards (RCOT/HCPC) and ensure safe, high-quality practice. Support complex cases and act as a point of contact for clinical queries. Monitor and evaluate your work through audits and outcome measures. Provide supervision, training, and support to colleagues and students. Train others in the safe use of equipment. Keep up to date with best practice, especially in frailty care. Take part in training, service development, and quality improvement. Step in for senior staff or team lead when required. Recognise when to escalate issues outside your role.
