Summary

PA required to support in accessing the community, the client does not like to be overloaded with information as this can become overwhelming. The client lives very much in the present and struggles if talking about something in the future, e.g. talking in the morning about what to have for lunch, instead of at lunch time. The client can easily become overwhelmed by noise, lights, crowds, so busy places can cause a sensory overload.

Preferred days/times:
Monday and Fridays, approximately 5 hours on each day, after 11am – there needs to be some flexibility in this.

Tasks/duties:
-To support to access the community and engage in the client’s choice of activities
-Social interaction
-Support the client to keep themselves and others safe in the community

Client main diagnosis:
Autism, Learning disability, photo sensitive epilepsy, bipolar disorder, hay fever, eczema and right hemiparesis.

Communication: Time needs to be given to the client along with simple, clear sentences without metaphors, allowing them to process information being given. If they feel overwhelmed the client can become agitated. If the client is tired, sometimes they will use picture cards to support communication. The client can make simple, basic choices.

Moving and Handling: The client is fully mobile, however, if the client feels unwell, this maybe an epileptic seizure and will need to sit down safely and calmly support her when she comes round.

Pets: No pets, the client is allergic to animal fur.

Client enjoys:
The client likes going to the library, shopping, going to the temple, going for lunch and would like to go to different places ie birdland.
