Summary

• Provides expert advice, support and interventions to patients of the anticoagulation service and to healthcare professionals in primary and secondary car
• Provides comprehensive evidence-based care for patients taking anticoagulant
• Uses computer dosing system to monitor and adjust doses for warfarin patients
• Prescribes as an independent non-medical prescriber for anticoagulant patients
• Supervises anticoagulant services support staff

· Monitor INR and adjust warfarin doses in face-to-face clinics across both Calderdale and Kirklees and some remote office working.

· Train and support patients who wish to self-test.

· Ensure patient safety: Consider and interpret VTE risks, where appropriate arrange for bridging of low INR’s and reversal of high INR’s. This may involve conducting VTE risk assessments, prescribing; liaising with colleagues in primary care and arranging referral to secondary care.

· Provide support and supervision for Higher Healthcare Assistants for domiciliary visiting. This may include remotely supervising the administration of medicines.

· Provide VTE and anticoagulation advice to trust staff for warfarin in-patients.

· Continually monitor standards of care and participate in service improvements to ensure the delivery of safe and effective care.

· Contribute and support departmental audits

· Identify clinical issues and incidents within the Trust that reduce the quality of care within the specialist service.

· Provide specialist education to members of the anticoagulation team.

· Participate in multi-professional formal and informal teaching to update staff on evidence-based anticoagulation practice.

· Work within the NMC Code of Professional Practice, and within policies, procedures and guidelines of Calderdale and Huddersfield NHS Foundation Trust.
