Summary
Day to Day Duties to include, but not exhaustive: Toprovideremote advice for patients withlong standingconditionslinked to why they are being remotelymonitoredby theLuscii To support the patient by linking in with their GP Practice Input into holistic care plans,toensure a focus on the individuals goals and support them tocomply withthe planultimately improvinghealth outcomes Patient education and health promotion Work toattainthe service KPIs and targets Ensure all patient contact is recordedin a timely manneron the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service To ensure mandatory training is kept up to date Acknowledge and accommodate barriers to communication and understanding including speech, hearing,languageand emotion. Flexibility in attending organised educational updates and eagerness to expand on current knowledge base and cascading new educational information to colleagues whenever possible To attend local practices to promote the home monitoring service as well as any relevant meetings and educational events.
