Summary

Able to use an advanced level of professional accountability, autonomy and judgement for seeing treating and assessing patients who present with a broad range of situations and conditions. Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients. Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgment for highly complex decision making across a broad range of situations. Have a level of understanding of PCN and NHS values, policies and procedures. Provide support to Care Home staff which may include delivering training where required, or supporting Care Home pilots, and support the other members of the Enhanced Care Team.
