As a Social Prescriber in our Health and Wellbeing team, you’ll help prisoners take meaningful steps towards improving their physical and mental health through personalised support and purposeful activity pathways.
You’ll create a positive impact by helping individuals build the skills, connections, and confidence they need to manage their wellbeing, engage with services, and shape a healthier future. Your work will touch lives every day and contribute to stronger, more supportive communities within the prison environment.
What you’ll do as a Social Prescriber
You’ll be entrusted to:
- Promote social prescribing and its role in self‑management and the wider determinants of health.
- Be a friendly and accessible source of information about wellbeing, prevention, and wider support services.
- Support prisoners to assess their health and wellbeing needs and develop personalised wellbeing plans.
- Connect individuals with appropriate services, programmes, and purposeful activities.
- Work collaboratively with healthcare colleagues, mental health professionals, and peer supporters to ensure coordinated care.
- Track progress, maintain accurate records, and adjust wellbeing plans as needed.

You’ll be a great fit for this role if you have…
- NVQ Level 3, Advanced Level or equivalent qualification, or are working towards it.
- Experience supporting people in a related role, paid or unpaid.
- Strong communication skills and the ability to build trusted, person‑centred relationships.
- An understanding of the wider social, economic, and environmental factors that impact wellbeing.
- Experience working collaboratively with partners, agencies, or community organisations.
Vetting and checks: An Enhanced DBS check is required for this role. This includes spent and unspent convictions, cautions, and any relevant information held by local police, in accordance with the Rehabilitation of Offenders Act and safeguarding requirements. You will also complete an Occupational Health Questionnaire to allow us to provide any support needed for you to succeed in the role.
