Summary
Working collaboratively with a multidisciplinary team, you will manage a diverse mix of telephone, face-to-face, and home visit appointments, delivering both planned and urgent primary care services. Your expertise will directly contribute to improving the health and well-being of the local community. Provide tailored, patient-centred care to meet individual needs. Proactively support patients with complex or long-term conditions to improve their quality of life. Offer expert advice and guidance to support the wider multidisciplinary team (MDT). Uphold high standards of evidence-based practice to ensure patient safety and satisfaction.
