Responsibilities
Report to The Registered Manager & Associate director of Quality and Governance.
It will be your responsibility to ensure that Care Plans are fully completed and compliant in line with the company policies, procedures and protocols along with the CQC standards. Carrying out a thorough outcome based initial assessment of clients needs and formulate a care plan that is person centred and aims to promote independence and achieve agreed outcomes, identifying the clients needs, wishes and preferences, ensuring any risks are identified and risk assessed appropriately.
Ensuring the client is at the centre of the assessment process and significant others e.g. family members, social workers, OT’s and GPs are involved and consulted as required or requested by the service user and conducting and recording risk assessments in line with the Lotus Home Care policies.
The Care Plan Assessor will be responsible for all new care package referrals, from the Local Authority, Commissioning Care Group (CCG), private referrals or any referrals from any third parties.
The Care Plan Assessor will be responsible for all care plan reviews including 12 months and ad-hoc assessments that are required within the branch. The Care Plan Assessor will actively monitor and record all care plans and reviews as per Lotus Home Care policy and procedures.
Skills, experiences and qualifications
Excellent organisational, administration and dairy management skills
Excellent communication skills
Strong IT skills including Excel, Word and good typing speed
Proven background in delivering high quality customer care/services
Experience in writing care plans
Experience in detailed risk assessments
Ability to work independently
Ideally previous carer/care industry experience
Ideally qualified to NVQ level 2 or higher in a relevant health and social care discipline
You must have good IT, communication and office skills
Hours of work
The usual hours of work will be Monday to Friday. This role will require you to be flexible for 8 hours per day, the usual hours of operation can vary and we require flexibility within our hours of operation, to be able to deliver care with the needs of the business.
