Summary
Seeking an experienced Locum Social Worker to join our busy Hospital Discharge Team, supporting safe and timely discharges through the Discharge to Assess (D2A) process. The role involves working closely with the multidisciplinary team to complete Care Act assessments, Mental Capacity Assessments, and strengths-based care planning for adults preparing to leave hospital.
You will coordinate discharge arrangements across D2A pathways (1, 2, and 3), ensuring patients receive the right support in the most appropriate setting while longer-term needs are assessed outside of hospital.
Key Responsibilities
Support safe, timely hospital discharges in line with D2A pathways and local discharge policy.
Complete Care Act assessments, Mental Capacity Act assessments, and Best Interest decisions where required.
Develop person-centred discharge plans and coordinate community support, reablement, or short-term placements.
Work collaboratively with MDT colleagues, patients, and families to ensure clear and effective discharge planning.
Identify and manage safeguarding concerns in line with statutory duties.
Maintain accurate and timely case records and manage a fast-paced caseload.
Essential Requirements
Qualified Social Worker registered with Social Work England.
Experience in hospital discharge, reablement, or adult social care.
Strong understanding of the Care Act, Mental Capacity Act, D2A pathways, and safeguarding procedures.
Ability to work autonomously and effectively within a multidisciplinary environment.
