Summary

Key Duties and Responsibilities Clinical Practice Deliver holistic, patient-centred care in accordance with NHS standards and practice protocols Undertake general nursing duties including wound care, dressings, injections, and venepuncture Undertake general nursing duties including wound care, dressings, injections, and venepuncture Administer Childhood and Adult immunisations and vaccinations in line with national schedules Perform cervical screening where qualified Contraception and Sexual Health Advice Carry out health checks (e.g. NHS Health Checks, new patient checks) Support the diagnosis, monitoring, and management of long-term conditions (e.g. diabetes, asthma, hypertension) Provide travel health advice and vaccinations (where trained) Health Promotion & Patient Education Promote health and wellbeing through lifestyle advice (smoking cessation, diet, exercise) Support patients in self-management of chronic conditions Contribute to public health initiatives and screening programmes Administration and professional responsibilities Participate in the administrative and professional responsibilities of the Practice team Ensure that accurate notes of all consultations and treatments are recorded in the patients notes Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit Attend and participate in Practice meetings as required Clinical Governance & Quality Work within the Nursing and Midwifery Council (NMC) Code of Conduct Maintain accurate, contemporaneous patient records using NHS clinical systems (e.g. EMIS, SystmOne) Participate in audit, quality improvement, and risk management activities Adhere to infection prevention and control policies Ensure safeguarding procedures are followed for vulnerable adults and children Team Working Work collaboratively with GPs, healthcare assistants, pharmacists, and other professionals Contribute to multidisciplinary team (MDT) meetings Support and mentor junior staff or students where appropriate Professional Development Maintain NMC registration and revalidation requirements Engage in continuous professional development (CPD) Undertake relevant training (e.g. immunisations, chronic disease management) Person Specification Essential Criteria Registered Nurse with valid registration with the Nursing and Midwifery Council Experience in a primary care or community setting (or transferable skills) Competence in core nursing skills (e.g. wound care, injections) Strong communication and interpersonal skills Ability to work autonomously and within a team Desirable Criteria Non-medical prescribing qualification preferred but not essential. Experience managing long-term conditions (e.g. diabetes, asthma) Cervical cytology and immunisation training Knowledge of NHS systems (EMIS, SystmOne) Understanding of Quality and Outcomes Framework (QOF) Values & Behaviours (NHS Aligned) Commitment to patient-centred care Compassion, respect, and dignity Accountability and professionalism Continuous improvement and learning Working Conditions Based in modern primary care setting May include extended hours or weekend clinics Exposure to clinical environments and infection risks Communication: Provide and receive highly complex, sensitive or contentious information and occasionally deal with situations which are hostile or emotive. Make effective use of the clinical system by ensuring consistent and accurate data entry and use of Read Codes Facilitate the use of up-to-date information technology to collate accurate and timely information as and when required by the Practice.
