Summary
As Deputy Director of Transformation (Planned Care), you will work closely with commissioning, contracting, procurement, finance, clinical and operational teams to ensure that transformation is evidence-based, coproduced with partners, and aligned to both national priorities and local needs. This role will also play a pivotal part in helping the Health and Care Partnership move toward a fully integrated health organisation with a population-based funding model. You will be: Providing strategic and day-to-day leadership of planned care transformation across the HCP. Designing and delivering new models of care and pathway redesign. Using population health insight and complex data to drive decision making. Ensuring transformation aligns with national policy, local priorities, and commissioning frameworks. Working collaboratively with providers, local authorities, PCNs, and the voluntary, community and social enterprise sector. Leading and supporting a developing transformation team. Promoting best practice, innovation and digital enablement. Ensuring value for money while improving outcomes for patients and communities.
