About the Role
Lambeth Council is seeking an experienced Qualified Social Worker to join the Intermediate Care Lambeth Rehab & Reablement Team. You’ll work within an integrated multidisciplinary service supporting adults receiving therapy and time‑limited reablement.
Key Responsibilities
Complete strength‑based assessments and statutory reviews
Evaluate progress against therapy goals and determine ongoing care needs
Work closely with OTs, Physios and allied health professionals
Attend MDT meetings and contribute to data collection for service improvement
Undertake Care Act assessments, safeguarding work and MCA assessments
Support hospital discharge pathways and admission avoidance
About You
Registered Social Worker (Social Work England)
Minimum 2 years post‑qualifying experience
Background in Intermediate Care, Reablement, hospital discharge or integrated teams
Strong assessment, analytical and recording skills
Confident working in a fast‑paced, multidisciplinary environment
