Summary

To support the primary care liaison team in investigating queries in relation to eligibility for inclusion on the GP Register and the sharing of information with colleagues within the wider liaison team and IDD SPA. To respond appropriately to urgent and non-routine situations as delegated. To participate in team meetings to contribute to the development and effectiveness of the service. To ensure confidentiality is always maintained. To work independently and as part of a Team. Support the Enhanced Service verification process with PCLN Colleagues. Signpost patients to supportive physical health interventions accessible to the service user. To work with the wider MDT and feedback patient care. To take an active role in assisting patients to take care of their physical health and understand the importance of good physical health in relation to their mental health. Under the guidance of the Qualified nurse, liaise with wider MDT and wider health community when advocating for individual needs of the patient. To have a good understanding and access of IT Systems including patient recording in SystmOne/EMIS and the Rio system. Support with the National Screening programmes and assist the PCLN Nurses with bowel screening calls and similar screening work. Support with blood desensitization work. Undertake physical health screening where appropriate for example weighing using electronic wheelchair scales and basic observations as required and record and report back to Qualified Nurse. Assist with collating patient feedback and letter to patients. To undertake any other reasonable duties as requested by the Team Leader. The role will include working across different bases across the city and county of Nottinghamshire and have a base within Clifton Cornerstone. The role will include some lone working. The post holder will at times visit patients within their own home environments and other community settings. The post holder will work under the direction of the qualified nurse. Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
