Summary
Leadership: This role will consist of a minimum of 80% operational leadership Exercise leadership and judgment in the management of the Community Nursing Team to ensure good clinical practice and maintain high standards of care, and review staff practice at regular intervals. Responsible for directing and supporting staff in their clinical roles. Meet service demand by prioritising visits; applying contingency plans; flexing service resources and escalating to senior management in crisis situations. Communicate highly complex information using informal and formal means to ensure people are fully informed about the service. Identify patient and service needs and proactively manage these to ensure effective service delivery. Support the Team Lead to maintain CQC standards Ensure that appropriate statutory, mandatory and essential training has been undertaken by all team members. Ensure service integration and collaboration with all key stakeholders to deliver the service, acknowledging the complexity of differing priorities. Act as a role model and demonstrate a willingness to lead and motivate a team, always providing clear and consistent leadership. To support the Team Lead by contributing to the development of team morale and motivation through effective personal leadership and communication with senior staff, team meetings and the promotion of clinical supervision for all staff. Responsible for the health, safety and welfare of self and others and to always comply with the requirements of health and safety regulations. Support the teams to raise alerts and undertake related activities with respect to safeguarding. Ensure structures are in place for good communications and flow of information and actions. Establish and maintain effective communication and liaison with the Primary Care Team and other agencies involved in caring for patients i.e. social care, acute trusts, voluntary and statutory agencies, relatives and informal carers. Effectively communicate at all levels of the organisation; to a variety of health and social care professionals working in primary, community and secondary care, out of hours providers, voluntary organisations and patients and carers. Provide high quality written reports and any other written documentation as necessary. To enter patient data on the EMIS system in a timely manner using where necessary the Team Planner, Daily Diary and Progress notes. Work closely with the organisation, integrated teams, key stakeholders, and partners. They will promote a service which works within local and national directives. Manage day to day capacity and operational planning, including rota management on behalf of the teams. Ensure the service complies with all relevant policies and procedures that relate to GESH. Participate in the Community out of hours on call rota Contribute to the business planning process. Clinical: This role will consist of a minimum of 20% clinical patient facing activity. Maintain defined competencies in core areas, as identified in the Clinical Lead competency framework. Confidently and competently assess and provide interventions to patients that require urgent intervention to avoid hospital admission e.g., End of Life, blocked catheters. Support teams to coordinate and prioritise the management of clinical caseloads with reference to patients with highly complex care needs. Provide clinical support, leadership, and guidance to ensure that evidence-based care and clinical practice are maintained in the community nursing team. Work in collaboration with the Primary Care Team and statutory and voluntary agencies to provide care for patients and their families and informal carers in their own home, within given resources. Support the teams to teach patients to self-manage their condition where possible and empower carers to actively participate in the treatment plan. Support the prevention of avoidable hospital admissions and promote healthier lifestyles. Effectively triage referrals into the service. Support teams to provide information to patients, their carers both formal and informal and families so they can make informed choices about current and future care needs. Support teams in conjunction with the patient/carer to develop a personalised care plan that reflects individual needs, preferences and choices and provide a Person Held Record for those patients with complex needs. Service Development: Support in the delivery of specific programmes of professional development. Encourage a culture of lifelong learning and development. Support staff in achieving and maintaining competencies and expected standards relevant to their banding. Support staff to participate in professional groups set up for the development of policies, protocols, standards, audit and development of the Community Nursing service. Continuing Professional Development: Responsible for keeping professionally updated and registered with the NMC. Always Work according to the NMC Code of Conduct and professional practice and the relevant legislative framework and exercise professional accountability Manage own personal development and ensure clinical credibility through on-going professional development as well as maintain a professional portfolio that demonstrates professional competence. Maintain Nurse Prescriber competencies and training requirements. Maintain compliancy with statutory, mandatory and essentials training requirements. Access appropriate and relevant training opportunities that will enhance both personal and professional development. Participate in clinical supervision and appraisal process. Participate in and lead on audit and research as required. Continuously review practice and implement change as appropriate and defined within the trust policy and service specification.
