An exciting opportunity has arisen for a number of motivated and enthusiastic Registered Nurses with current NMC Registration to join a dynamic multidisciplinary Urgent Response/Home IV team/ Hospital and Care At Home Virtual Ward Team.

The team manages acutely unwell patients at home using a variety of pathways e.g IV antibiotics via cellulitis pathway , DVT pathway we are currently developing further pathways to manage patients in their own environment and prevent unnecessary admissions to hospital e.g frailty pathway.

HOPT ( OPAT) facilitates discharge and manages patients at home with long term antibiotics in Salford and Out of area.

This is expanding to include Hospital and Care at Home (H&C@H) virtual wards model of care working as part of a team utilising technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier than may have been possible previously.

The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Virtual Wards pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of Salford and deliver an NHS national priority.

You will be responsible for providing nursing care and intervention, with an aim to help adults to continue to live at home. You will be a competent autonomous practitioner who will need to be lone working at times with the support of senior members of staff on shift.

You will be working as apart of a large multidisciplinary team that includes Physiotherapy, Occupational Therapist , Social Workers, Advanced Practitioners and Care Support Workers.

We take referrals for medical and/or social crisis from the community and Salford departments, aiming to promote early discharge from hospital and avoid unnecessary hospital admissions; by implementing treatment/care plans in patients own home.

The Service operates a 7 day service, currently covering the hours of 8am until 10pm, including weekends. You will be expected to work shifts to cover these hours, and need to be flexible.

Due to the nature of the post it is expected that you will have the ability and means to travel to different locations across the area using your own car.

This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

The NCA can no longer accept applications from International Nurses who are yet to complete all competencies needed to receive a full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.
