Summary


We are looking for an experienced Registered Mental Health Nurse or Registered Learning Disabilities Nurse to join our dynamic and committed team on Lister at The Wells Road Centre.

Lister Ward is a 12-bedded male learning disabilities ward. Multi-disciplinary, structured approaches to risk assessment and management are a key feature of the service and through our assessment of patient needs / risks we will establish appropriate care pathways for each patient. A range of psychological, occupational, educational and vocational approaches will be available to support the patients with their mental health, and from low secure to community accommodation. As such, there will be an emphasis on developing links with community services and promoting social inclusion.

To actively support the Ward Manager in fulfilling all the Key Result Areas and Responsibilities associated with their Clinical/Management Role. To assess, implement, and evaluate care programs for individual patients, providing professional knowledge and skills.
Practices which foster and develop multi-disciplinary working will be implemented in all aspects of clinical practice.

The role requires a RNLD or RMN Nurse with current registration with the nursing and Midwifery council who will deliver nursing care to patients in line with specified standards within a secure clinical setting.
. To deputise in absence of a senior manager.
. To lead a team of staff within low secure mental health provision.
To promote and maintain high standards of care through communication, motivation, supervision and role modelling.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

To assume responsibility for co-ordinating clinical care for the ward and act as a role model for members of the team.
-Build and maintain sound therapeutic relationships with patients and their families and carers.
- Organise and supervise the pre-admission assessments, admission, transfer and discharge of patients.
-To undertake Unit Co-ordinator responsibilities as directed by Ward Manager / Clinical Nurse Specialist.
-To regularly take charge of the ward and deputise in the absence of the Ward Manager.
-Accept professional responsibility for the delivery of care.
-To lead a team that promotes and maintains the highest standard of care at all times, assuming supervision of staff at Band 5 and below.
- Assist with setting and monitoring of clinical standards within the immediate clinical area, ensuring measurable outcomes are achieved.
-Accept and participate in regular supervision with Ward Manager.
-Facilitate clinical supervision with members of the nursing team.
- Keep the Ward Manager informed of any pertinent matters relating to patient care.
- When in charge ensuring that all necessary information is collated and related to other necessary personnel and appropriate action taken to resolve the situation.
-Develop a high degree of autonomy and responsibility while remaining within the overall clinical direction of the ward and accountable to the service manager.
- Demonstrate an understanding of and support the principles of advocacy.
-Actively promote an awareness and understanding of issues surrounding equality and diversity.
-Maintain strict confidentiality of patient information at all times.
-Delegate fairly to staff, appropriate to their ability and the workload of the unit.
- Familiarise and carry out procedures in accordance with laid down Trust policies and local procedures, e.g., fire, health and safety.
- Maintain good order and cleanliness on the ward in line with infection control procedures and audit.
-Maintain safe custody of medicines and their administration in line with national/local policies and guidelines.
- Contribute to a systematic individualised care planning approach to patients care with the involvement of allied disciplines and modern nursing strategies.
- Maintain a safe environment for patients and staff.
- Be fully conversant with the provisions of the Mental Health Act 1983, Discharge and After Care Policies including: - the Care Programme Approach.
-Ability to work with other disciplines to maximum extent and develop skills in a variety of inputs.
