Summary
You will: Deliver specialist social work assessments and interventions in both inpatient and community settings. Provide compassionate, person centred support to patients, families and carers navigating complex emotional, practical and safeguarding needs. Offer expert guidance to colleagues on social work practice, legislation and safeguarding. Work collaboratively with nurses, doctors, counsellors, chaplaincy and other professionals to ensure coordinated, safe and effective care. Build strong relationships with external agencies to advocate for individuals rights, wishes and outcomes. Contribute to service development, quality improvement, education and innovation within the hospice.
