Summary


**Community Mental Health Nurse (Bank)**

We are looking for Bank Community Mental Health Nurses to join the Stepney and Wapping Neighbourhood Mental Health Team.

This role involves being an active participant in our multidisciplinary team, providing assessments and interventions to individuals within the Stepney and Wapping community mental health services.

The post holder will also deliver brief sessional support to mental health service users through our Duty System. A key aspect of this role is to focus on helping service users achieve their recovery and social inclusion goals, through both individual and group work. Additionally, there are care coordination responsibilities. The success of the individual in this role will be evaluated based on the outcomes and satisfaction of service users, as well as the extent to which occupational therapy input helps users transition away from needing secondary mental health care support.




• To act as a care co-ordinator responsible for managing a caseload of service users in the community, and ensure the provision of high quality and evidence based mental health care and incorporating Care Act Assessment when required.
• To assess, plan, implement and review care in accordance with CPA and trust policies.
• To undertake carer’s assessments and provide services, information and support to carers as appropriate.
• To develop and implement health promotion strategies relevant to the service user and carer group.
• To ensure principles of risk Assessment and risk management are followed and implemented in the provision of care.
• To conduct comprehensive mental health assessments and to develop coproduced care and support plans with service users and carers accordingly.
