Summary
You will be responsible for: Clinical Care: Accepting referrals, conducting patient-centred assessments, and delivering structured interventions tailored to individual needs. You'll also educate and involve family members and carers where appropriate. Communication & Collaboration: Working closely with clinical colleagues and external services to ensure seamless care pathways. You'll attend multidisciplinary meetings and contribute to service development. Supervision & Development: Engaging in regular case management and line management supervision, attending relevant training, and maintaining professional registration. Governance & Quality: Adhering to clinical governance standards, safeguarding policies, and data protection regulations. You'll contribute to continuous improvement through feedback and reflective practice. Administration & Data Management: Using Trust systems (e.g. RiO, IAPTus) to record clinical activity, monitor outcomes, and support service performance reporting.
