We work closely across the Nursing teams, including our Community Rapid Response Service, Unplanned Care, Podiatry, Physio, Falls Team, Stroke and Rehab teams whilst also liaising with our Palliative Care team and Local Hospices to ensure that our patients are at the centre of the care provided and have access to the services they need when they need them.
