Summary
To deliver a nursing service that provides high quality care to patients in line with clinical and regulatory requirements. To assess plan and deliver holistic care while supporting patients and caregivers to manage health conditions independently and confidently. To maintain regulatory compliance and communicate effectively with internal and external stakeholders.Key AccountabilitiesClinical Care Delivery Assess plan and deliver high quality evidence based care to patients Manage a caseload of patients with varying clinical needs Complete comprehensive patient assessments Develop individualised care plans and review them regularly Administer medications treatments and therapies as prescribed Monitor vital signs and perform clinical assessments as required Demonstrate sound clinical judgement and risk based decision making Escalate changes in patient condition appropriately Respond effectively to emergencies or sudden deterioration Provide culturally sensitive person centred care Work collaboratively with multidisciplinary teams Maintain and develop clinical skills and knowledge Recognise limitations of competence and seek support when necessary Participate in on call services where applicablePatient and Caregiver Education Provide clear education on disease and medication management Offer guidance and resources to support lifestyle modification Ensure patients and caregivers feel confident in medication management Identify environmental risks and recommend safety improvements Teach signs of complications such as infection or adverse medication effects Act as an advocate for patients and families Provide emotional support and reassuranceRegulatory Compliance and Clinical Governance Adhere to all clinical excellence policies procedures and professional standards Report incidents near misses safeguarding concerns and adverse events Maintain accurate timely and complete patient documentation Participate in clinical audits and complete related actions Carry out risk assessments and contribute to service reviews Ensure patient confidentiality at all timesCommunication and Collaboration Communicate effectively with patients families and healthcare colleagues Provide clear updates and clinical information to physicians and teams Explain complex information in a clear and accessible way Work with Nursing Service Managers to identify risks and improvement opportunities Build and maintain positive stakeholder relationshipsEnablers to the Role Skills Knowledge Experience Active Registered Nurse with current NMC registration SACT Passport Clinical experience in Oncology Proficiency in clinical procedures including IV administration medication management and patient assessment Ability to make autonomous decisions and manage unexpected challenges Strong verbal and written communication skills Ability to build rapport and trust with patients and families Respect for diverse backgrounds beliefs and values Knowledge of safety procedures for in home or on site care Accurate and timely clinical documentation Adaptability to varying environments and schedules Ability to teach patients and caregivers effectively Full valid UK driving licence
