Summary
To work as an effective member of the broader Oakhaven Community nursing team, undertaking holistic palliativenursing assessments, delivering hands on nursing care and supporting both patients and their families in their home or place of care in the community. To work as a professional resource and educator for health and social care professionals, as appropriate, within the Oakhaven catchment area. To provide regular hands on delivery of care to patients primarily within their own homes, or place of care including care homes where necessary. Assess, plan and implement plans of care based on person centred palliative care best practice principles that reflect patient and family assessed need and in partnership with them. Actively including patients and families in decision making. To promote and actively support patients to achieve their preferred place of care, and preferred place of death by providing effective and timely nursing care , including rapid assessment and urgent nursing care interventions when required. To act as an education support for health and social care professionals patients and families with the Oakhaven catchment area, using the principles of situated learning to develop palliative and end of life care skill, knowledge and confidence in others Ensure, along with other team members, the provision of skilled high quality evidence based nursing care to patients and their families. Maintain a holistic approach incorporating psychosocial, spiritual and social aspects of care for patients and families. Ensure effective communication both within Oakhaven teams, Oakhaven Hospice and Oakhaven Care and externally. To work collaboratively with the wider MDT within Oakhaven Hospice. To act as clinical support, both practical and nursing advice, for the Oakhaven Care Palliative Home Care Team in line with the broader Oakhaven Community Team To maintain excellent documentation, recording and data collection. To participate in audit activity in line with the Oakhaven audit programme. Utilise and develop own teaching skills Participate in the delivery of both formal and informal education as appropriate, in any setting. Develop and maintain own professional clinical skills and knowledge. To exercise professional responsibility by accessing and accepting opportunities for reflection and relevant training, in order to understand and respond to the dynamics of providing excellent palliative and end of life care. To be mindful of the limited resources available within the community and promote cost effectiveness. To work flexibly across the clinical directorate to meet patient need.
