Summary
Job Purpose: To provide a high-quality service to the patients of the Practice; diagnosing and prescribing treatments and drugs while working within defined procedures and protocols, under the support and supervision of the GPs. Clinical Duties and Responsibilities: 1.Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs 2.Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities 3.Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs 4.Act as part of the clinical governance team for the Practice and ensure significant events are reported using Practice procedures 5.Develop a lead role in an area of special interest 6.Undertake home visits as needed including to vulnerable patients in our care homes and those who are housebound 7.Take part in practice meetings including clinical team meetings, general education sessions, multidisciplinary team meetings (including those with outside agencies as needed) 8.Make use of Advice and Guidance as needed in the course of patient treatment and care 9.Follow up on lab results with patients and agree appropriate treatment plans and priorities. AdministrationDuties and Responsibilities: 1.To maintain accurate patient records and enter onto the computer using agreed templates and coding 2.To collect data and participate in regular Practice audit and research projects 3.To contribute to the preparation of any Practice development plans GeneralDuties and Responsibilities: 1.To maintain own 3 yearly registration with the NMC 2.To adhere to the NMC Code of Professional Conduct 3.To maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co-workers from other agencies. Ensure that all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to 4.To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice 5.To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice 6.Attend and participate in staff meetings 7.Any other reasonable duties that may be required from time-to-time as commensurate with the role
