Summary
We are seeking motivated, dynamic, and passionate Band 5 Community Nurses to work within our South Bedfordshire Primary Home Care Team, based in Dunstable, however, there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. This post is with a community team that predominantly see palliative patients, so an interest in palliative care is essential. You will be expected to see other community caseload patients as well to support the wider teams as required. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Youmusthave a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.
