Summary
Be an integral member of the integrated community team and, under the direction of the qualified clinician, provide care and rehabilitation for patients in their own homes or clinics. This care will be based on the qualified professionals assessed need for each patient and will be carried out to the agreed standards taking account of Trust policies, procedures and guidelines.
