Summary

The postholder will work as an autonomous Paramedic Practitioner as part of the practices urgent care and same day access team. A core element of the role is leading the development and delivery of a safe and effective clinical triage system for the practice. Working closely with the GPs, Practice Manager and multidisciplinary team, the Paramedic Practitioner will support the efficient management of clinical demand and provide high quality assessment and treatment for patients with acute presentations.Key Duties and Responsibilities 1. Clinical Triage & Demand Management -Lead the design, implementation and ongoing development of the practices formal triage system. - Establish and coordinate a triage hub, including workflow design, standard operating procedures and integration with reception and clinical teams. - Provide clinical triage for same day / urgent patient requests and allocate care based on acuity and clinical need. 2. Urgent Care & MAC Clinics -Work alongside the duty doctor to deliver MAC clinics and manage urgent on the day presentations. - Assess, diagnose, treat and appropriately escalate patients presenting with acute illness or injury. - Provide accurate safety netting, follow up advice and contribute to maintaining smooth workflow during peak demand. 3. Home Visits -Undertake urgent home visits for acute presentations as required, providing timely assessment and management within the community setting. 4. Multidisciplinary Team Working -Work collaboratively with GPs, nurse practitioners, other clinicians and administrative teams to support continuity of care and maximise clinical capacity. - Participate in case discussions, handovers and clinical meetings as appropriate. 5. Clinical Governance & Service Improvement -Contribute to the evaluation and continuous improvement of urgent care pathways, triage processes and access systems. - Engage with practice audits, quality improvement projects and safety initiatives. 6. Professional Responsibilities -Maintain HCPC registration and meet all continuing professional development (CPD) requirements. - Work within professional boundaries and scope of practice, seeking senior clinical support where appropriate. - If qualified, practise as an independent prescriber; if not yet qualified, be willing to undertake prescribing training.
