Summary

Job Summary The Advanced Nurse Practitioner (ANP) works autonomously within the GP practice, providing advanced, evidencebased clinical assessment, diagnosis, treatment, and management for patients presenting with a wide range of acute, chronic, and complex conditions. The postholder will independently prescribe, undertake clinical investigations, interpret results, and develop comprehensive management plans in line with national and local guidelines. The ANP plays a key role in improving patient access, supporting safe clinical decision making, and contributing to the ongoing development of high quality primary care services. The role requires strong leadership, excellent clinical judgement, and commitment to multidisciplinary working to ensure continuity of care and optimal patient outcomes. Purpose of Job The Advanced Nurse Practitioner (ANP) works as an autonomous, highly skilled clinician within the GP practice, delivering advanced patient assessment, diagnosis, treatment, and follow up care. The purpose of the role is to enhance clinical capacity, improve access, and provide high quality, patient centred care for individuals presenting with acute, chronic, and complex conditions. The ANP is expected to use advanced clinical reasoning, independent prescribing, and expert judgement to manage undifferentiated presentations and support continuity of care. The postholder contributes to improving clinical outcomes, reducing GP workload, and strengthening the practices ability to meet demand safely and effectively. The role incorporates leadership, service improvement, and the professional development of the wider multidisciplinary team, supporting excellence in patient care, clinical governance, and continuous improvement across the practice. Accountable to the Practice Board of Directors to provide a timely and responsive service of all business, administrative, clinical and patient services. Clinical Responsibilities Provide advanced clinical assessment, diagnosis and management of patients with acute, chronic and complex conditions Independently prescribe medications in line with national guidance and practice policies. Request and interpret diagnostic tests such as bloods, ECGs and imaging where appropriate Manage long-term conditions in line with local and national clinical pathways. Provide urgent/on-the-day care and triage according to clinical risk. Deliver health promotion, disease prevention, and self-care education Undertake home visits and care home reviews where required. Maintain accurate, contemporaneous clinical records. Contribute to QOF, DES/LES and other quality initiatives. Leadership, Quality & Governance Uphold the NMC Code and ensure safe, evidence based clinical practice Participate in audits, quality improvement and significant event reviews Support safeguarding responsibilities for both children and adults. Ensure safe prescribing, medicines optimisation, and infection prevention. Participate in mandatory training, appraisal and revalidation. Multidisciplinary Team working Work closely with GPs, clinicians, pharmacists, nurses and the wider MDT Liaise with community teams, secondary care and external agencies Support escalation for complex cases and provide clinical advice to colleagues Education & Development Act as a clinical resource for the wider team Support training, induction and supervision of junior staff or trainees Engage in ongoing CPD and maintain advanced practice competencies Participate in peer review, clinical supervision and reflective practice General Duties Provide clear and concise information to patients. Put patients at ease and give them the opportunity to explain their needs. Employ a variety of questioning techniques to develop a full picture of the nature of the patients requirements. Identify potentially serious problems and bring those to the immediate attention of the appropriate clinician. For less serious problems, identify and agree an appropriate timeframe for the patient to be seen, with the appropriate clinician. Build and maintain strong relationships with all colleagues. Develop and maintain an in-depth knowledge and understanding of the services provided by the Practice and the wider health community. Demonstrate assertiveness without being domineering and have the ability to gain confidence of the service user. Assess own performance and take accountability for own actions, either directly or under supervision. Effectively manage own time, workload and resources. Any other duties that may be requested by the Doctors, Practice Nurses or the Practice Manager Maintain confidentiality and comply with GDPR regulations.
